School-based health services make going to the doctor as easy as boarding our mobile health unit located at the school. When students and staff are not feeling well it makes the school day less enjoyable. School-Based Health Services are extended on-site medical, dental, and behavioral/mental health services for students and staff within a respective school district. The objective of School-Based Health Centers is to give students access to care in a location that is safe and convenient. Onsite health care professionals collaborate with schools to address the broad range of concerns and adverse experiences that affect students and staff's healthy development.
At this time, Agape Health Services is happy to serve the Washington County School District at 5 different locations.
Individuals wishing to participate must be enrolled in SBHS program. Listed below are printable enrollment forms for school staff and students. Enrollment packets can be returned to the office personnel at each school.
